自2019年12月新型冠状病毒(2019-nCoV)感染的肺炎疫情发生后,国家卫生健康委员会组织相关专家制定了《新型冠状病毒感染的肺炎诊疗方案》试行、试行第二版和试行第三、四版。其中均提到2019-nCoV肺炎具有一定的特征性胸部影像学表现:早期呈现多发小斑片影及间质改变,以肺外带明显。进而发展为双肺多发毛玻璃样密度影(GGO)、浸润影,严重者可出现肺实变,胸腔积液少见。本指南由我国传染病影像学专委会专家制定,推荐2019-nCoV肺炎的首选和主要的成像技术是高分辨率CT(HRCT),根据病变范围与类型将CT表现分为早期、进展期、转归期与重症期。早期影像学表现为局限在胸膜下分布的斑片状、亚段或节段性GGO,伴或不伴小叶间隔增厚;进展期影像学表现为病灶进展,范围增大,病灶增多,累及多个肺叶;转归期影像学表现:病变吸收或纤维化;重症期影像学表现:双肺弥漫性实变或GGO,呈“白肺”表现。胸部X线检查早期多无异常表现;进展期可呈支气管炎伴局限性斑片影;或重症期X线表现为双肺弥漫性多发实变。胸部X线检查漏诊率高,不推荐使用。2019-nCoV肺炎与其它病毒性肺炎,如流感病毒、副流感病毒、腺病毒、呼吸道合胞病毒、鼻病毒、人偏肺病毒、非典型肺炎(SARS)冠状病毒等引起的肺炎影像学表现上重叠,需结合接触史、旅游史,实验室检查,才能得出准确诊断,也需要与机化性肺炎、嗜酸性肺炎等非感染性病变鉴别。为增进广大医务工作者对新型冠状病毒肺炎这一疾病的认识和理解,传染病放射学专委会主任委员李宏军教授牵头组织全国各定点收治医院的医学影像学专家共同紧急制定“新型冠状病毒肺炎影像学诊断指南(第一版,试用版)”。
