  研究共收集具有HBV感染表现的人群共988例,其中包括慢性HBV感染者伴肝硬化的429例、无肝硬化的慢性HBV感染者285例和274例健康恢复者。确定研究人群中肝硬化及其并发症的发生、肝病进展程度和速率、HBV标志物等临床表现型;运用基质辅助激光解吸附电离飞行时间质谱法检测9个候选基因上的14个单核苷酸基因多态性(SNP)位点的基因型,分析候选基因SNP与肝硬化发生及严重程度、肝病疾病进展速率的关系。

  结果发现HBV感染的中国人群呈现不同的疾病进展程度和速率。位于AZIN1基因的rs2679757位点及TRPM5基因的rs886277位点与肝硬化发生危险相关,rs2679757 GG+AG基因型和rs886277 CC基因型与较重的肝硬化程度、较快的疾病进展速率和高肝硬化失代偿发生率相关。

  研究提示AZIN1和TRPM5基因多态性可能影响慢性HBV感染患者肝纤维化的进展,有待进一步机制探讨;不同的人种(如高加索人群和中国人群)等位基因多态性的分布和频率具有较大差别;不同肝病可能具有不同的发生机制和遗传影响因素。需更大样本和全基因组关联研究探讨中国人群、乙肝肝硬化危险相关遗传因素并进一步进行基础和临床验证,以建立中国人群乙肝患者特异的硬化发生危险标志。
