JAD:痴呆前的这一阶段,或可通过简单补充维生素B改善认知!

PON1是一个与认知能力受损有关的新因素,可以通过B族维生素改善MCI患者的认知能力。

识别影响认知能力下降的可改变的风险因素对于制定预防和治疗策略非常重要。对氧酶1(PON1)是一种高密度脂蛋白相关的酶。PON1基因产物,也叫血清对氧磷酶,不仅在有机磷神经毒剂解毒中起重要作用,而且与动脉粥样硬化冠心病2型糖尿病甚至神经系统疾病(包括阿尔茨海默病)等疾病发病有密切关系的发展中起作用。

多年前,英国科学家在最新临床实验中发现,每天服用大剂量的维生素B可能使大脑萎缩的速度减慢一半,从而延缓老年痴呆症的病程。为此,近期有科学家假设,即PON1状态可预测轻度认知障碍(MCI)患者的认知能力并设计了相关研究,结果发表在Journal of Alzheimer Disease杂志上。

研究人员将参加随机、双盲安慰剂对照试验(VITACOG)的MCI患者(n = 196,76.8岁,60%为女性)被分配接受每日剂量的叶酸(0.8毫克)、维生素B12(0.5毫克)和B6(20毫克)(n = 95)或安慰剂(n = 101),为期两年。认知能力通过神经心理学测试进行分析。通过MRI对一部分参与者(n = 168)的大脑萎缩进行了量化。通过实验室检测PON1状态,包括PON1 Q192R基因型。

结果显示,在安慰剂组中,PON1的基线苯乙酸水解酶(PhAcase)活性(而不是对氧酶活性或PON1 Q192R基因型)与整体认知(MMSE;TICS-m)、言语偶发记忆(Hopkins Verbal Learning Test-revised: 总回忆,HVLT-TR;延迟回忆,HVLT-DR),以及研究结束时的注意力/处理速度(线索制作A和符号数字模式测试,SDMT)有关。

POase与PhAcase的群体分布图显示了根据PON1 192QQ、PON1 192QR、PON1 192RR基因型的MCI个体分布率。

除PhAcase外,基线铁和甘油三酯可预测MMSE,基线脂肪酸可预测SDMT,基线抗N-Hcy-蛋白自身抗体可预测TICS-m、SDMT、Trail Making A,而BDNF V66M基因型可预测研究结束时HVLT-TR和HVLT-DR的分数。其中,B族维生素能消除PON1和其他变量与认知的联系。

综上,PON1是一个与认知能力受损有关的新因素,可以通过B族维生素改善MCI患者的认知能力。
