J Endod：CBCT示根尖周炎但X线无显示患牙的临床因素：一项回顾性临床研究

共有332名拍摄了CBCT和PA X线影像的患者纳入此项研究。回顾性分析患者的临床病例以收集诊断和症状信息。由2名牙髓专科医师通过改良CBCT PA指标评估根尖周损伤。评估患者相关因素（年龄、性别和症状）和牙齿相关因素（牙齿类型、部位、牙髓状态和牙髓诊断）以检查它们与仅在CBCT影像上存在的根尖周炎之间的关系。

结果显示，通过PA X线影像和CBCT检测的未治疗牙齿根尖周炎的检出率分别为24.6%和35.5%。多变量逻辑回归分析结果显示，仅在CBCT影像上可见的根尖周炎与牙髓坏死显著相关(odds ratio = 5.401； 95% confidence interval， 1.911-15.265； P = .001)；对所涉及到的磨牙观察发现，边界线不具有明显的关联性(odds ratio = 2.843； 95% confidence interval， 0.990-8.164； P = .052)。损伤大小直径在2 mm以内和磨牙是损伤仅在CBCT影像上可见的重要因素。

结论：牙髓坏死是仅在CBCT影像上可见根尖周炎的术前预测因子。这篇研究为CBCT影像在诊断阶段的应用提供了适当的指示。
