J Endod：鼻内酮咯酸和笑气联合应用对有症状的不可逆牙髓炎患者下牙槽神经阻滞麻醉效果的影响

有研究报道，不可逆牙髓炎患者在麻醉前应用酮咯酸会增加下牙槽神经阻滞麻醉(IANB)的成功率。同样，提前给予笑气也会增加IANB的成功率。近来，已研发出可鼻内传导的酮咯酸。那么，联合酮咯酸和笑气可能会更有效的提高IANB的成功率。因此，这项前瞻性随机双盲试验是通过对罹患有症状的不可逆牙髓炎患者联合应用酮咯酸和笑气/氧气，评估其对IANB麻醉成功率的影响。

有研究报道，不可逆牙髓炎患者在麻醉前应用酮咯酸会增加下牙槽神经阻滞麻醉(IANB)的成功率。同样，提前给予笑气也会增加IANB的成功率。近来，已研发出可鼻内传导的酮咯酸。那么，联合酮咯酸和笑气可能会更有效的提高IANB的成功率。因此，这项前瞻性随机双盲试验是通过对罹患有症状的不可逆牙髓炎患者联合应用酮咯酸和笑气/氧气，评估其对IANB麻醉成功率的影响。研究选取102名罹患有症状的不可逆牙髓炎并具有中度到重度疼痛的患者下颌后牙作为评估对象。将患者随机分为2组，并在给予笑气/氧气前实施鼻内酮咯酸31.5 mg或盐水安慰剂20分钟。在施加笑气/氧气后10分钟，进行IANB。在嘴唇深度麻木后进行牙髓治疗。麻醉成功的标准：能够执行牙髓治疗以及无痛或轻微疼痛的探查根管。结果显示，鼻内给予盐水/笑气组与鼻内酮咯酸/笑气组对于IANB成功率的比值比为1.631，并且组间未见明显差异(P = .2523)。结论：与单独使用笑气/氧气相比，麻醉前应用酮咯酸并不能显著增加IANB的成功率。追加麻醉依旧需要以获得足够的麻醉效果。原始出处：Stentz D， Drum M， et al.
