三尖瓣环周围室早心电图特征

1、Ⅰ导联QRS波均为正向(R或r型);

2、aVL导联QRS波多为正向,少数有负向起始波;

3、三尖瓣环游离壁室早QRS波宽度大于间隔部室早;

4、游离壁起源室早肢体导联QRS多可见“切迹”;

5、V1导联显示游离壁起源多为rS型,间隔部多为QS型;

6、游离壁起源室早胸前导联QRS波移行多在V3以后。

心内电生理检查特点

1、导管位置判定方法:右前斜及左前斜位透视,消融导管特殊运动;消融导管记录到小A大V波形,A/V<1,且A波振幅>0.03mv,V波振幅>0.35mv。

2、导管消融途径分为常规途径和特殊途径,前者为经右侧股静脉,后者为经锁骨下静脉、颈内静脉或经动脉消融(左室侧)。

3、异丙肾上腺素有助于室早的诱发,激动标测时,局部心室激动较QRS波提前30ms左右,起搏标测有助于快速确定室早起源点。窦性心律下,消融成功部位局部A波振幅0.2±0.2mv;V波振幅2.2±1.3mv。
