J Dent Res:颞颌关节盘移位的女性患者机械行为评分与正常人之间的比较

软骨疲劳可能是发育早熟的颞下颌关节(TMJ)发生退行性性变的影响因素。这项横向研究是通过TMJ能量密度(ED)和颚肌的占空因数(DF)相联合的方式来计算评估因软骨疲劳造成女性双侧TMJ盘移位(DD)的机械行为得分(MBS)与正常值之间的差异。

软骨疲劳可能是发育早熟的颞下颌关节(TMJ)发生退行性性变的影响因素。这项横向研究是通过TMJ能量密度(ED)和颚肌的占空因数(DF)相联合的方式来计算评估因软骨疲劳造成女性双侧TMJ盘移位(DD)的机械行为得分(MBS)与正常值之间的差异。

所有受试者均知情同意自愿参与此项目,并按照诊断标准(DC)诊断为颞下颌关节紊乱症(TMD),同时通过核磁共振(MR)和CT成像进行观察。47名患者按照诊断标准分为DD阳性(n=29)和DD阴性(n=18)。动态比重测定法(MR影像结合咬合轨迹数据)用来记录TMJ应力场力学的个性化数据,并在开闭口循环(下颌骨闭合时的咀嚼负荷为20-N)均匀进行10次的过程中测定ED值(ED = W/Q mJ/mm3, W=任务完成, Q=软骨体积)。受试者通过训练记录超过3天3夜的咀嚼肌和颞肌的肌电图。阈值的确定是通过均根方肌电图/咬力标准来记录在每20-N咬力(T20 N, ?V)下具有受试者特异性的咀嚼肌和颞肌的活动。肌肉DF(DF=% 肌肉活动期间/整个记录时间)是由阈值的范围所确定,MBS= ED2 × DF。ED、DF和MBS的组间差异采用方差分析及Bonferroni、Tukey honest significant difference post hoc tests进行统计学测定。

结果显示,与一侧DD阴性相比较,对侧TMJ DD阳性的受试者平均ED值要显著增大( P = 0.012),为DD阴性的1.4倍。DD阳性患者的平均DF值要明显大于DD阴性的受试者(all P < 0.01),DD阳性患者在白天为DD阴性的1.7倍,晚上为DD阴性的2.5倍,整体为DD阴性的1.9倍。DD阳性患者的日间MBS要显著大于DD阴性受试者(all P < 0.04),为DD阴性的8.5倍。

结论:双侧TMJ DD女性的ED、DF和MBS值要显著大于正常女性。
