Stroke:血清胱抑素C水平与脑小血管疾病之间的关系

近日,心血管疾病领域权威杂志Stroke上发表了一篇研究文章,该研究旨在探讨血清胱抑素C水平与脑小血管疾病(CSVD)及其严重程度之间的关系。

该研究从2017年到2019年纳入了中国丽水市的社区居民,他们来自多血管认知障碍和血管事件评估(PRECISE)队列研究的横断面调查。基线调查时,研究人员评估了受试者CSVD总负荷和改良的CSVD总负荷评分,以及磁共振成像检查上CSVD的标志物,包括白质高强度、腔隙性卒中、脑微出血和血管周围间隙。

根据胱抑素C的四分位数将参与者分为4组。研究人员使用顺序或二元Logistic回归模型分析血清胱抑素C水平与CSVD总负担和影像学标志物之间的关系。此外,研究人员采用2样本孟德尔随机化分析探究了胱抑素C水平对CSVD的遗传预测效应。

该研究共纳入了3061名参与者。参与者的平均年龄为61.2±6.7岁,其中1637名(53.5%)参与者为女性。经协变量调整后,胱抑制素C水平升高与CSVD总负荷增加和CSVD总负荷相关(Q4与Q1:总比值比[OR]为1.30[95%CI为1.03-1.64]和1.32[95%CI为1.01-1.73])。此外,与半胱氨酸抑制素C水平的前四分位相比,后四分位的受试者出现腔隙性卒中(OR为1.99[95%CI为1.05-3.76])、修饰性白质高强度负荷(总OR为1.42[95%CI为1.07-1.90])和中重度血管周围间隙(OR为2.15[95%CI为1.29-3.59])的风险更高,但没有出现脑微出血。孟德尔随机分析显示,基因预测较高的胱抑制素C水平与腔隙性卒中风险增加相关(OR为1.16[95%CI为1.06-1.27])。

由此可见,在这项以社区为基础的研究中,研究人员发现胱抑素C水平与CSVD之间可能存在关联,尤其是对于腔隙性卒中,而这与估计的肾小球滤过率无关。
