Sci Rep：左西孟旦在心脏手术左室功能不全患者中的使用

目前没有足够的高质量证据支持或禁止在心脏手术LVD患者中使用左西孟旦。

接受心脏手术的左室功能不全（LVD）患者死亡率高。左西孟旦，一种钙增敏剂，可以改善心肌收缩力，而不增加心肌耗氧量。目前关于左西孟旦是否能降低接受心脏手术的LVD患者死亡率尚不清楚。

研究人员对PubMed、Embase和Cochrane中心数据库进行检索，以查找在心脏手术LVD患者中比较左西孟旦与常规治疗的随机试验。使用随机效应模型导出汇总风险比（RRs）。主要终点是围手术期死亡率。次要终点是肾脏替代治疗、房颤、心梗、室性心律失常和低血压。

纳入15项研究，共2606例患者。左西孟旦降低围手术期死亡率（RR：0.64,95％CI：0.45-0.91）和肾替代治疗发生率（RR：0.71,95％CI：0.52-0.95）。然而，敏感性分析、亚组分析和试验序贯分析（TSA）显示需要更多的证据。此外，左西孟旦没有降低房颤（RR：0.82,95％CI：0.64-1.07）、心梗（RR：0.56,95％CI：0.26-1.23）或室性心律失常（RR：0.74,95％CI：0.49-1.11）的发病率，但增加了低血压的发病率（RR：1.11,95％CI：1.00-1.23）。

目前没有足够的高质量证据支持或禁止在心脏手术LVD患者中使用左西孟旦。
