NEJM:播散性球孢子菌病-病例报道

该患者开始抗真菌治疗,给予脂质体两性霉素B和氟康唑的组合2周,然后进行氟康唑单一疗法。HIV基因型检测显示患者对服用的一种药物具有抗药性,并且调整了抗逆转录病毒治疗方案。

患者为一名有人类免疫缺陷病毒(HIV)感染史的34岁的男性,因头痛、发热和精神病史1周而到急诊就诊。

他的体温为39.3°C,在体检时,舌头上发现大的溃疡性病变(如图A所示)。患者的CD4计数为每立方毫米39个细胞(参考范围为500至1450),他的HIV病毒载量为每毫升197000个拷贝。胸部X线片显示两肺均有斑片状浸润。在血液和脑脊髓液(CSF)中检测到针对球孢子菌的IgG抗体,并且在血液、CSF和尿液以及在支气管肺泡灌洗液中获得了的液体中检测到球孢子虫抗原。获得舌病变的活组织检查标本,并用苏木精和伊红染色显示多种真菌生物与球孢子菌小球一致(如图B所示)。在支气管肺泡灌洗液中获得的液体进行真菌培养物也检测到了球孢子菌。

在就诊后3个月的随访中,患者的发热和头痛已经消退,舌头病变缩小,并且HIV病毒载量变得无法检测到,随后患者失访了。
