Mol Psychiatry:外周炎症也能入脑!通过检测外周可以预测老年痴呆风险

神经性炎症竟与阿尔兹海默病发病直接相关

痴呆症是造成老年人残疾和依赖的主要原因之一,因为它影响到记忆、思维、行为和进行日常活动的能力。目前,全世界有超过5千万人患有痴呆症,估计到2050年这个数字将接近1.52亿。除了开发有效的药物,一些可改变的风险因素可以作为潜在的手段来减轻人口老龄化带来的日益增长的疾病负担。

最常见的阿尔茨海默病(AD)通常在 65 岁以后发病(迟发性AD)。最常见的与迟发性AD相关的基因是一种叫做载脂蛋白 E(APOE)的风险基因。其中,APOE e4会增加患AD的风险。

炎症通常是机体应对损伤和压力所产生的反应,比如机体伴随受伤或感染而出现的红肿表现等;然而,大脑中的炎症(神经性炎症)如今被认为与多种疾病直接相关,比如抑郁症、精神病和多发性硬化症等,而且最近研究人员发现神经性炎症竟与阿尔兹海默病发病直接相关

中枢免疫成分尤其是小胶质细胞在痴呆症中的作用已被充分研究,相应的免疫疗法也逐渐引起了人们的注意。然而,很少有研究关注外周免疫和痴呆症。为了解决这个问题,来自复旦华山神经内科的医生开展了相关研究,结果发表在Molecular Psychiatry杂志上。

利用英国生物库(UKB)361653名参与者的数据,研究了痴呆症事件与外周免疫标志物之间的纵向联系,包括免疫细胞计数及其衍生比率,包括中性粒细胞对淋巴细胞比率(NLR)、血小板对淋巴细胞比率(PLR)、系统免疫炎症指数(SII)和淋巴细胞对单核细胞比率(LMR)。

在8.99年的中位随访期间,4239名参与者患上了痴呆症。结果显示,先天免疫标记物的增加与较高的痴呆风险有关(每SD增量危险比[HR];95%CI: 中性粒细胞为1.14;1.09-1.19,NLR为1.16;1.11-1.20,SII为1.11;1.07-1.16),而适应性免疫标记物的增加与低痴呆风险有关(淋巴细胞为0.93;0.90-0.97,LMR为0.94;0.90-0.98)。

研究指出了先天性和适应性免疫在痴呆症发病中的不同作用,这可能为痴呆症的病因和治疗提供一些新的视角。
