Neurology病例:自身免疫性胶质纤维酸性蛋白脑膜脑脊髓炎

73岁骨髓增生异常综合征女性,表现为认知和运动能力下降2月,伴有头痛和体重下降。查体提示患者思睡,对称齿轮样肌强直,运动迟缓,上臂肌阵挛和反射亢进。

73岁骨髓增生异常综合征女性,表现为认知和运动能力下降2月,伴有头痛和体重下降。查体提示患者思睡,对称齿轮样肌强直,运动迟缓,上臂肌阵挛和反射亢进。脑MRI可见软脑膜,线样/点状脑实质和室管膜强化(图1)。脊柱MRI证实无强化的颈胸段脊髓信号改变,脊髓圆锥环形强化伴中央管强化(图2)。脑脊液呈炎性改变,胶质纤维酸性蛋白(GFAP)-免疫球蛋白G(IgG)阳性。患者对静脉注射甲强龙反应迅速。GFAP-IgG相关自身免疫性脑膜脑脊髓炎是一种严重但对皮质类固醇激素治疗反应很好的疾病,具有独特的MRI表现。
